You will receive a confirmation email from iris@growingartscenter.com shortly. Payment is due to secure your child's spot. After filling out this online form make your check payable to Growing Arts and put it in the after- school mailbox (on the right hand wall as you enter the Duniway office). Pick up time is 4:00. Mark your calendar for our class's informal performance at 3:30pm on June 4th for family and friends.

I understand that I Dance is an active class and will not hold Iris Nason or Growing Arts liable for any physical injuries that may occur during or after class. I recognize the possibility of physical injury inherent to dance and hereby assume the risk connected with any participation by my child in any movement class with Iris Nason or Growing Arts.
I consent to have photos and videos taken of my child for promotional material for Growing Arts and I Dance during class. I understand that these photos and videos may be used in print and on the Growing Arts website and Growing Arts Facebook page without compensation. Names of children in photos will not be used.
